2. What type of personal information does Holistic Wellness Advantage & Melody Byblow collect?

a. Melody Byblow o/a Holistic Wellness Advantage collect various types of personal information. We may collect any of the following: Your name, billing/shipping address, email address, telephone number, and credit card information, including card number, card type, cardholder name and expiration date, when you purchase products or services from us or products from the online shop. We may also obtain some or all of this information when you create a user account through, for example, Kartra or Practice Better websites, or a secure encrypted payment processor such as Stripe, within Holistic Wellness Advantage & Melody Byblow accounts. By purchasing a product or service from us or opting in to view a masterclass or download other content you consent to have your email address added to a our emailing subscriber list with the purpose of updating you on your purchase, delivering virtual products, informing you of upcoming news, events, updates, promotions and contests pertaining to Melody Byblow and Holistic Wellness Advantage.

b. Personal information you provide to us when you participate in any of our on-line programs, other meeting spaces, blogs or forums, or book a call. This personal information might include your name, profession, e-mail address, phone number, photograph, answers to specific questions, password and computer information. The personal information you provide to us when participating in our online programs, meeting spaces, blogs or forums may be used for marketing purposes. Please be cognizant of any personal information you post in our internet forums, as we cannot guarantee that all other participants will respect your privacy.

c. Personal information you provide to us when you participate in one of our promotions, surveys or contests. This information may include your name, email address and street address, and the answers to survey questions. The personal information you provide to us when participating in these promotions, surveys and contests may be used for marketing purposes.

d. Personal information you provide to us when subscribing to our newsletters or emails or when you use our website, including, but not limited to when you create a profile in Holistic Wellness Advantage, or, for example, our Accounts on Kartra or Practice Better. This personal information might include your name, billing/shipping address, e-mail address, telephone number, and credit card information, including card number, card type, cardholder name and expiration date.

e. Your personal information from our business partners and other third parties provided they have agreed to obtain your consent before disclosing your personal information to us.

f. Certain technical information from your computer each time you request a page during a visit to our web site www.holisticwellnessadvantage.ca or the Kartra Account or Practice Better Account. This information is collected from your computer’s Web browser to enhance your experience on our web site and may include your IP address, operating system, Web browser software, screen resolution, and referral website.

3. How does Melody Byblow o/a Holistic Wellness Advantage collect personal information?

We collect personal information that is voluntarily provided to us by our customers, members, sponsors, website users and the public in general when they contact us via telephone, e-mail, web browser, or in person. As mentioned above, we may also collect personal information from our business partners and other third parties who have agreed to obtain the consent of the person the information pertains to before disclosing the information to us. We may also collect personal information through the following means:

a. E-mail subscriptions. We may collect first name, last name, phone number, address, and e-mail address from anyone who chooses to sign up or opt-in to download free resources or register for a masterclass video.

b. Surveys. We collect survey information that is voluntarily provided.

c. Web beacons. To assess the effectiveness of promotional and marketing e-mails we send, we may attach a web beacon to the email that provides us with confirmation when the recipient opens the e-mail. This only occurs if the recipient’s computer supports such technology.

d. Nutrition consultation intake forms and sharing of other personal health information, such as lab reports. All health information provided via e-mailed and uploaded intake forms will be communicated through an encrypted HIPAA-compliant email server within our Practice Better Account. Your use of that portal to communicate with us helps ensure your private information is secure. Please refer to the Privacy Policy on Practice Better.

4. Why does Melody Byblow o/a Holistic Wellness Advantage collect your Personal Information?

We may collect personal information about you and use it for the following reasons:

a. To provide products and services to you, to inform you of changes and other relevant information pertaining to your purchase, to bill you when you make a purchase from us, to acknowledge your support, and to provide that information if challenged legally by a government agency for having permission to contact you when necessary for these purposes.

b. To provide you with services or recognition you request.

c. For marketing purposes. We may use your contact information to send you emails about promotions, surveys, contests and special events or providing other marketing information to you. Promotional e-mails are intended to inform you of new or unique opportunities, events or special deals that are available from Melody Byblow o/a Holistic Wellness Advantage. You can opt-out and choose not to receive these emails from us by either clicking “unsubscribe” at the bottom of the email when you receive it or by sending an email that includes your e-mail address and a request that you not receive our promotional e-mails. Melody Byblow o/a Holistic Wellness Advantage may also prepare aggregated anonymized user statistics or information summaries to describe our services and their popularity to our business partners and prospective advertisers and for other lawful purposes. The aggregated user statistics and information will not contain any personally identifiable information.

g. To provide a secure and safe environment for our visitors and employees.

h. To co-operate with law enforcement and to protect against violations of criminal or civil laws.

We may use your personal information to respond when you submit a question or suggestion to us, or when you request assistance with your membership, viewing your purchase(s), logging on to your account in our portal on, for example, in Kartra, or Practice Better, or www.holisticwellnessadvantage.ca.

5. THE DISCLOSURE OF PERSONAL INFORMATION

Melody Byblow o/a Holistic Wellness Advantage takes your privacy very seriously and WILL NEVER sell or rent any personal information we collect. We may disclose some personal information we collect to service providers who help us provide personalized content and advertising to you. Such as when you buy a product through our shop, Melody Byblow o/a Holistic Wellness Advantage discloses your personal information to a third-party service provider for the financial transaction, fulfillment, servicing, and/or shipping of your order. We cannot ensure that each of our service providers to whom you share personal information are committed to maintaining the privacy of such information as outlined in this Privacy Policy, and their own Privacy Policies will then apply. 

6. HOW WE PROTECT YOUR PRIVACY

a. Melody Byblow o/a Holistic Wellness Advantage is committed to protecting your privacy. Security measures, such as locked filing cabinets, restricted access and the use of passwords and encryption have been adopted to protect your personal information against loss or theft, as well as unauthorized access, disclosure, copying, use or modification.

b. Please ensure that any emails including personal health information are done through the encrypted HIPAA-compliant email service via the Practice Better website Account for Melody Byblow o/a Holistic Wellness Advantage in your portal.

c. All information submitted to our websites or email addresses is transferred to the Melody Byblow o/a Holistic Wellness Advantage computer servers.

d. Melody Byblow o/a Holistic Wellness Advantage will keep your personal information only as long as it remains necessary or relevant for the purposes set out in this Privacy Policy or as required by law.

e. There are several places throughout our website that may link you to other web sites that do not operate under Melody Byblow o/a Holistic Wellness Advantage Privacy Policy. When you click through to these web sites, Melody Byblow o/a Holistic Wellness Advantage Privacy Policy no longer applies.
